    Laboratory Considerations for HbA1c Testing in the Presence of HbF are very important. How can labs address the challenges of elevated fetal hemoglobin (HbF) in HbA1c testing? Accurate diagnostics depend on understanding interferences: • Boronate affinity and immunoassays are sensitive to HbF interference, requiring careful result interpretation. • Ion Exchange HPLC analyzers not only measure HbA1c but also identify HbF peaks, offering insights into potential interferences. Laboratories using cation-exchange HPLC can detect HbF. With Tosoh G8 elevated HbF levels of up to 25% do not affect HbA1c results. Beyond this threshold, the system flags the presence of HbF and refrains from reporting an HbA1c value. The International Diabetes Federation (IDF) 2024 reports that over 537 million adults are living with diabetes—a number expected to grow. There are three main types of diabetes—Type 1, Type 2, and gestational diabetes—but one of the most effective tools for managing all of them is the Hemoglobin A1c (HbA1c) test. This simple test measures average blood glucose levels over 2-3 months and is key to early detection: • A1c levels ≥ 6.5% indicate diabetes • 5.7-6.4% signals prediabetes Unchecked, diabetes can lead to serious complications like heart disease, kidney failure, and vision loss.
