Did you purchase a restaurant or other food service in 2024? You must submit a change of ownership form. You MAY NOT cross out the license holder information on a license renewal form. Also, if you no longer hold the license for a facility, you must contact the health department to put the facility out of business, and new owners must submit the proper forms for a new business. You can find those on our website: https://ow.ly/ZlNg50UNUni
