    We are excited to highlight our upcoming course, The Velopharyngeal Port: The Forgotten Sphincter in Pediatric Feeding and Swallowing, taught by Loredana Cuglietta MHSc., R.SLP., S-LP(C). This course will explore critical aspects of velopharyngeal function in infancy and childhood. Loredana will discuss the developmental trajectory of velopharyngeal function, the implications of nasopharyngeal reflux as a potential indicator of aspiration, and the differentiation between structural and neuromotor velopharyngeal dysfunction (VPD). Participants will gain insights into effective strategies for assessing and addressing both types of VPD, empowering them to optimize care for pediatric patients.     Today we are discussing the Dynamic Imaging Grade of Swallowing Toxicity (DIGEST). Let's get into it! The Dynamic Imaging Grade of Swallowing Toxicity (DIGEST) is a validated system used to assess the severity of pharyngeal swallowing impairment due to cancer. With the development of DIGEST, MBS rating was adapted to the NCI’s Common Terminology Criteria for Adverse Events (CTCAE) nomenclature of ordinal toxicity grading used in oncology trials (Hutcheson et al., 2017). DIGEST employs a decision tree or flowsheet to assist clinicians in evaluating standard radiographic modified barium swallow studies (Hutcheson et al., 2022). “DIGEST offers a psychometrically sound measure for HNC clinical trials and investigations of toxicity profiles, dose-response, and predictive modeling” (Hutcheson et al., 2017). DIGEST-FEES is a reliable scale for assessing the severity of safety, efficiency, and overall pharyngeal stage swallowing impairment in people with head and neck cancer (HNC) using FEES (Starmer et al., 2021). DIGEST-FEES aligns with the original DIGEST scale, demonstrating strong correlations with reference measures of swallowing function, in addition to other FEES assessments. Further, it has moderate correlations with other dysphagia measures, including patient-perceived quality of life, diet level, and secretion severity. Interested in learning more?     Today we are reviewing information regarding the upper esophageal sphincter (UES). Let's get into it! The Upper Esophageal Sphincter (UES) spans a 4-cm segment along the digestive tract, comprising cartilaginous, bony, and muscular components. It operates through a combination of intrinsic muscular control and displacement, facilitating its opening and closing (Ramaswamy et al., 2022). The UES, characterized as a high-pressure zone, can be observed through manometry. The anatomical components comprising this zone are known as the PES. The terms UES and PES are synonymous. According to Lang & Shaker (1997) the UES/PES is made up of the cricopharyngeus (CP), inferior pharyngeal constrictor (IPC), and the cervical esophagus. "Recent basic science investigations have suggested that the upper esophageal sphincter (UES), in cross-section, is not round, but that it more closely approximates a kidney shape" Belafsky et al.
