Signature Health is proud to receive $71,000 in grant funds from the Three Arches Foundation to continue patient navigation and transportation services for high-risk patients in Lakewood and surrounding communities. Three Arches Foundation is a 501(c)(3) public charity that invests in advancing the health and well-being of the people of Lakewood and surrounding communities through grantmaking. Enjoyed our visit this morning with the team at Signature Health, Inc. as we delivered our final 2024 Annual Grant Award to continue patient navigation and transportation services for high-risk patients in Lakewood and surrounding communities. Congratulations again to all 2024 recipients and to four additional nonprofit partners who are carrying their impactful work into year two through multi-year grants awarded in 2023 — LifeAct of Northeast Ohio, NUEVA LUZ URBAN RESOURCE CENTER, Colors+ Youth Center, and In Harmony Therapeutic Services.

Despite the cold and snowy conditions, we delivered 2024 Grant Awards to five more nonprofit partners today. Congratulations to Planned Parenthood of Greater Ohio, Neighborhood Family Practice, Bellefaire JCB, Cleveland Clinic, and GiGi's Playhouse Cleveland. Thank you for the work you do every day to improve equitable access to behavioral and physical health care.

On this Giving Tuesday, we’re proud to support the work and grateful for the efforts of our nonprofit partners as they advance their missions, strengthen community bonds, and inspire transformative change. Today we delivered seven more 2024 grant awards to Partnership for Good Health, B RILEY SOBER HOUSE, Eliza Jennings, Urban Community School, Re:Source Cleveland, May Dugan Center, and Spanish American Committee. Smart Development is beyond thankful to the Three Arches Foundation for their support to help Newcomers understand and navigate the local health care system by bridging cultural and linguistic gaps through the work of a health and wellness team comprised of a social worker, case manager, and community navigator. Through this work, we hope to grow resilience in Newcomers across Greater Cleveland, enhance existing and build new community partnerships to compliment our work, and make Greater Cleveland an even more welcoming and healthy place to live.
