https://www.tembo.health Hi everyone, Wow- it has been an interesting couple of months as I have been researching the new 8-year Medicare GUIDE dementia program and trying to find out how we can bring this support to Minnesotan's living with dementia and their care partners. I want to be part of this program that I believe will change dementia care for our country and was inspired to be part of it as a Care Navigator. I am joining the team at Tembo Health Tele-health as a Care Navigator for their patients around the U.S. and an excited to get the GUIDE program going with Tembo in Minnesota. Do you have anyone who would be interested in the free Medicare GUIDE program? Please pass on this information to help them find me and I can assist them in starting enrollment. Do you provide services that I should know more about as a Care Navigator in Minnesota? I would love to connect and review your services. The Medicare GUIDE program covers: Complete Assessment by Tembo Health Physician (not to take the place of your Primary Physician) who is contracted with Medicare for the program Care Navigation- early enrolled patients will get curated resources and monthly check ins with me, Amy House. 24/7 Support Line with Tembo Health $2500 to use for respite services for the care partner/caregiver Caregiver support groups, skills training and more This change is bittersweet as I leave SilverCrest Properties as their Executive Director of Memory Care at Brightondale, but my families and residents have been so supportive. They understand how helpful it would have been to have me guiding them while their loved ones were still at home. The U of M Caregiving for People with Memory Loss conference last week was so good! Sara Barsel did a great job explaining our work with making air travel more dementia friendly and had wonderful tips to ensure safe and less stress travel. I saw the Justice Bus for the first time-what a neat idea!! Thank you to Carrie Puterbaugh LALD who helped at the table for Think Dementia and Silvercrest Properties!

The Dementia Friendly Airports Working Group DFAWG is featured in this article about our partnership with Teepa Snow creating videos for training or for those flying on how to have a more positive approach and offer the right amount of support. It was exciting to see the feature in the Special Year End Issue of Alzheimer’s Today Magazine. I am a founding member of this group and so pleased to see what we have accomplished in the past five years. Up next in Feb we will be doing a training at MSP Aiport in February to faculty of many different departments in which I have been creating the training with four other amazing women and will be one of the Trainers.
