    In the last week, I've spent 20+ hours on the phone with Anthem Blue Cross and Blue Shield. I am trying to get a spine surgery approved for my husband, Connor, who is fully disabled from a covid infection he acquired while working as a physician. His covid infection has left him with severe bilateral jugular stenosis, meaning both of his jugular veins are fully pancaked because of bones in his spine pushing on them. He has been suffering in darkness and silence for 14 months because of the severity of his symptoms. Anthem has approved the entire operation, but will not approve the CPT codes for the only two surgeons who do this surgery within hundreds of miles of where we live. We relocated via air ambulance (which we paid out of pocket for) to the east coast to be closer to these surgeons. His surgery is scheduled for Monday, October 21st and we are going to miss it because of Anthem's negligence. ?? I have taken three full days off from work to go after Anthem. ?? I have spoken with dozens of representatives who have routed me to non-working numbers to get me off the line. I call them right back. ?? I have escalated it to the California Department of Managed Health Care, a state regulatory agency that oversees health plans. Their urgent appeal process gives them 7 days to make a determination. ?? I have spoken with 50+ neurosurgery practices within 30 miles of my home to prove to Anthem that no other surgeons are doing this surgery (of course they know this but what's 50 extra phone calls). Nothing has worked. Anthem has played games beyond my wildest dreams. They file appeals incorrectly and there is no recourse for their errors. They just restart their timeline of urgent appeals and end the phone call. Health insurance should not get in the way of critical care. The fact that it is allowed to is abhorrent. We have fully failed as a democratized nation for allowing it to. Pic is from 2022, right after Connor first got sick but before things became what they've become. I miss this version of him. I miss seeing him in the light. I miss hearing his voice. I miss seeing him walk. I've been a full-time caregiver for three years. I am tired. I am exhausted to my core.     Why should physicians be concerned about patient engagement? According to the NIH, US hospitals experience on average $88 billion in missed revenue. This statistic is why so many hospitals close their doors annually. What can be done? The NIH claims that patient engagement is a clear indicator of health-improving behavior. These individuals are not victims of their circumstances but rather solution-finders and problem-solvers. The results: 1. Better outcomes 2.
