Mental compulsions are just as devastating as physical compulsions when it comes to interfering in the life of a person with OCD. While many people are familiar with OCD in the sense of what they see in movies or TV shows (i.e., people washing their hands, straightening things, and checking the locks or the stove), there is another side of OCD that is not often acknowledged - the mental compulsion. Sometimes referred to as Pure O, OCD with only (or primarily) mental compulsions is common and can be difficult to treat if you are not trained in identifying how mental compulsions manifest themselves. This workshop will focus on how to bring Exposure and Response Prevention (ERP) to bear on mental compulsions. Through a thorough review of the various subtypes of OCD, the presenters will give examples of the way mental compulsions can play a role in the maintenance of OCD and then discuss how to develop ERP exercises to specifically challenge those mental compulsions, to decrease the influence of OCD in the life of the sufferer. This presentation offers an alternative interpretation of patients’ apparent resistance, termed pathological ambivalence, which is rooted in early experience, biological functioning, and psychological narrative. These factors result in the development of strong but conflicting needs (such as wanting connection and fearing rejection) that can slow down, confuse, or even halt the therapeutic process. This workshop will offer a framework to utilize specific strategies from a wide variety of evidence based treatments that enable the patient to resolve the ambivalence thus minimizing the likelihood that the therapy and/or therapist will become the target of the ambivalence. Case examples and experiential learning will also be utilized to educate the participants to sidestep power struggles, projections and splitting and to empower clients to resolve ambivalence from within. Despite the diversity of content that brings clients to therapy, difficulty regulating their emotional experience is at the heart of their struggles. Clients can feel hijacked by extreme emotional states, uncomfortable in their own skin, or think or behave in ways they wish they wouldn’t. Polyvagal Theory (PVT) helps us understand what is happening on a biological level when our clients are emotionally dysregulated or stuck in adaptive survival states, such as fight, flight, freeze, or numb. Internal Family Systems Therapy (IFS) offers a compassionate, non-pathologizing approach to healing the wounded, burdened, and traumatized parts of clients’ systems and increasing internal harmony and connection. IFS is an evidence-based model of psychotherapy that provides a compassionate, respectful, non-pathologizing approach to understanding the organization and functioning of the human psyche.
