Responsibilities include offering hands-on assistance in daily living skills, implementing health and behavior support plans, facilitating community activities, and documenting progress. The role involves planning recreational and vocational opportunities, assisting with personal care and household tasks, providing transportation, and ensuring a safe, positive environment. Additional duties may include participating in team meetings, developing care plans, maintaining facility upkeep, and adhering to privacy regulations. The position also requires ongoing learning and communication with supervisors and other team members to ensure high-quality support. LOCATION Canandaigua, NY SCHEDULE Hiring for all hours; weekends; weekdays; day shift; night shift! DIFFERENTIAL $1.00 pay differential for night shift; $1.00 pay differential weekend shifts! RESPONSIBILITIES Provides hands-on assistance, instruction, and training to individuals which will enable them to develop and/or acquire skills in various aspects of daily living which may include communication, social, recreational, and leisure activities, self or personal care, household management, travel, work-related or vocational opportunities, money management, etc. in accordance with an individualized plan of service, and following all safety considerations. RESPONSIBILITIES - Acts as an agency contact/liaison for each assigned program participant.
