70% of the Global PAD Association's network polled shows patients were misdiagnosed when first presenting with textbook PAD symptoms of leg pain and leg cramps. So, we are launching a new series, Journey To Diagnosis, in partnership with Biomedix to explore the common misdiagnosis that lead to delays in PAD care. In this episode, Dr. Craig Walker, MD, discusses how symptoms of iliac artery blockages can mimic that of back issues. A Tale of Two Patients: Why We Need to Act Now on PAD in ESRD This past week, I encountered two stories that broke my heart and ignited my passion to create change: On the East Coast, a patient with end-stage renal disease (ESRD) came to us too late. Despite having an skilled vascular surgeon ready to review the case, we're likely losing the battle to save their leg today. The patient's leg pain and reduced walking distance weren't acknowledged soon enough to get timely vascular intervention. In Kansas City, another ESRD patient's journey ended in tragedy. Months ago, he reported pain while walking. Diagnosed with peripheral artery disease (PAD), he was told nothing could be done due to his ESRD and contrast dye risks. The advice? "Go to the ER when you can't walk at all." Two weeks ago, he did just that, knowing his fate was sealed. He chose a nerve block over amputation and passed away on February 10th. His wife called our Leg Saver Hotline after he died, wishing she had found us months earlier. We could have connected them with incredible vascular surgeons in Kansas City who've saved legs and improved mobility for our ESRD patients using advanced techniques like CO2 angiography instead of contrast dye. These stories haunt me. How does it get this far? Why aren't all chronic kidney disease patients tested for PAD before reaching ESRD? Why do some doctors give up instead of referring patients to specialists with advanced skills? We need change: 1. Early PAD screening for all CKD patients 2. Better awareness of PAD symptoms among CKD/ESRD patients and doctors 3. More training in advanced, kidney-friendly vascular techniques 4. A commitment to never give up on a patient's quality of life To the doctors reading this: - EARLY DIAGNOSIS: In the Journal of Vascular Surgery in 2013, Jean Bismuth, Houssam Younes, MD,FACS,FSVS,RPVI, et al shared a study that concluded, "PAD is under recognized in ESRD patients and risk factors are not being treated.?Screening and awareness of PAD in the ESRD/HD population should be enhanced." And it's still a big issue that's not being addressed sufficiently. We need to figure out how to get these patients diagnosed sooner. Hoping the ARC Act will pass and that reimbursement for all high-risk patients is passed and maybe that will pave the way for the change we all want to see. - EARLY TREATMENT: Once these patients are diagnosed, how to we make sure they don't fall through the cracks and end up with reduced mobility and wounds that won't heal? How do we ensure that those vascular surgeons who specialize in treating these patients with advanced care are given the chance to treat them vs being referred to those who say, "Nothing can be done?" Let's work together to ensure no more ESRD patients lose their dignity, their mobility, or their lives to undiagnosed PAD. Share this post. For those who see me fighting for life and limb on a daily basis, it may come across that I may be unaware of cases in which Amputation is treatment. Truth be told, I have supported and continue to support doctors and patients on a path to amputation, through amputation, and long-term management - when it's a case of life or limb and when a patient decides they want to move on with a different path to stop a revolving door of wound care, endovascular, and surgical treatments. In my posts, my goal is to raise the voice of the patients and their experiences to help create the change patients want to see. For Dawn, she knows it was her life or limb. She ended up with an AKA. It was not an easy pill to swallow for either she or her doctor. The decision was not made lightly. Both wish there was another way. But all efforts to save her leg were exhausted. She says that she hears stories all the time of other amputees who were not offered advanced limb salvage options prior to ending up on the 'chopping block' (her words!) So, she asked that I share this video as a reminder to all PAD practitioners, what life is like for an amputee, especially in the first 6-months to a year as their incision heals and prosthetics are fitted. She hopes it'll maybe inspire doctors to just take that extra opportunity to find a a way to help patients walk to live another day and live to walk another day.
