As the population of older adults in America increases and becomes ever more diverse, it is critical to strategically plan for their changing health needs at all levels of government, beginning with State Plans on Aging and Multisector Plans for Aging. These are designed to transform the available systems and support offerings to improve aging. While there are key differences, both have the same goal: to support all older adults age well. State Plans on Aging must be in place for a span of years to receive funding from Administration for Community Living. Multisector Plans for Aging typically span a decade, are voluntary, and can be used in conjunction with existing State Plans on Aging to transform available services and supports for older adults. Right now, there is no coordinated and easily navigated system in place to support Americans who want to live independently as they age. The time for solutions is now. Across all the work we fund, we value projects that are bold, catalytic, and impact-oriented. OUR AREAS OF FOCUS ? Transform care and delivery: This goal is to establish person-centered, integrated models as the gold standard of care that all older adults come to expect and receive—regardless of whether they have Medicare only, or are dually eligible for Medicare and Medicaid. ? Build resilience and capacity: This goal is to inspire and enable innovators to create new offerings that embrace the beauty of life as we age, and enrich the lives of older adults and families. We encourage innovators to design with older adults, not just for them. ? Drive responsive financing policies: This goal is to drive responsive federal and state financing policies to create meaningful care choices for older adults of today and tomorrow. HOW WE BEGAN The SCAN Foundation was created as an independent charitable organization in April 2008 through a one-time contribution of $205 million from the not-for-profit SCAN Health Plan.

A new The Washington Post story highlights the importance of social connection for older adults, and is part of our support of UC Berkeley Graduate School of Journalism. Jon Esse is one of 300 volunteers at Hero’s Bridge, a Virginia-based organization that helps older veterans navigate insurance, find housing, and access transportation to medical appointments. Jon volunteers for their “Battle Buddy” program, which connects older veterans with their younger veteran counterparts who understand the unique mental and physical tolls of military service. He spent many hours with John Victor, a 92-year-old Army veteran. Although Jon is 66 years old and qualifies for assistance from the program himself, he helped John V. with house repairs and yard work and much-needed company. As the workforce ages, supporting older workers isn’t just important—it’s essential. Without it, we risk losing invaluable expertise. An inclusive workplace benefits everyone. So, how do we ensure older adults continue to grow and thrive in this evolving labor landscape? The Stanford Center on Longevity in collaboration with CASBS at Stanford has launched their #FuturesProject on Education and Learning for Longer Lives to answer that very question. The new initiative brings together leaders from industry, VC, government, academic, and nonprofit sectors, including The SCAN Foundation’s own VP of Innovation and Investments, Anika Heavener. Over the next year, she and the other Future Fellows will carry this ethos forward as they work to shape a national vision that not only prioritizes vocational dignity but also explores innovative ways to leverage the valuable insights gained from combining lived and professional experiences. How can capital create the systemic change needed to truly enrich the lives of older adults? This critical question was explored at #SOCAP24, where this year’s theme was “Going Deeper: Catalyzing Systems Change.” The SCAN Foundation’s Vice President of Innovation and Investments, Anika Heavener, engaged with global thought leaders and contributed key insights at the SF-based Impact Economy event.
