The SCAN Foundation, led by President and CEO Sarita A. Mohanty, MD, MPH, MBA, is dedicated to creating a society where older adults can access health and supportive services of their choosing to meet their needs. To make this vision a reality, we work to ensure the availability, affordability, and quality of long-term services and supports that keep vulnerable older adults independent in the place they call home. WHY OUR WORK MATTERS As all of us grow older, most people will encounter daily challenges, such as: ? Getting help with daily needs like going to the grocery store and keeping the house clean; ? Managing chronic illness; ? Worrying about how to pay for care and services; and/or ? Wondering about being able to live in a place of one’s choosing. Right now, there is no coordinated and easily navigated system in place to support Americans who want to live independently as they age. The time for solutions is now. Across all the work we fund, we value projects that are bold, catalytic, and impact-oriented. OUR AREAS OF FOCUS ? Transform care and delivery: This goal is to establish person-centered, integrated models as the gold standard of care that all older adults come to expect and receive—regardless of whether they have Medicare only, or are dually eligible for Medicare and Medicaid. ? Build resilience and capacity: This goal is to inspire and enable innovators to create new offerings that embrace the beauty of life as we age, and enrich the lives of older adults and families. We encourage innovators to design with older adults, not just for them. ? Drive responsive financing policies: This goal is to drive responsive federal and state financing policies to create meaningful care choices for older adults of today and tomorrow. HOW WE BEGAN The SCAN Foundation was created as an independent charitable organization in April 2008 through a one-time contribution of $205 million from the not-for-profit SCAN Health Plan.

In response to Secretary Kennedy’s announcement of cuts and reorganization at HHS, President and CEO of The SCAN Foundation Sarita Mohanty, MD, MPH, MBA, shared: "Roughly 10,000 people in the United States turn 65 every day, at a time when the human and financial cost of caring for older adults is steadily increasing. What America needs now is a coordinated federal effort to make it easier for families to care for older adults and combat chronic disease. HHS’s Administration for Community Living is the only dedicated government agency that helps older adults age in their home and in their community, which is what people say they want. Any realignment of the agency or its functions must preserve the essential work the government has long done to improve the lives of older adults and ensure alignment with Medicare and Medicaid to improve and expand delivery of the services on which Americans rely." At The SCAN Foundation, we believe it’s critical to preserve and strengthen services that help older adults age in place. Yesterday, the Disability Rights California Gala in Sacramento brought together advocates, stakeholders, partners, and trailblazers in the disability justice space to celebrate the impactful work being done across California. ? Representing TSF, Director of California Health and Aging Policy Megan Burke, LCSW and Director of Community Impact and California Partnerships Vivian Nava-Schellinger attended in support of the movement to create equitable solutions for older adults. Anna Garcia, from the Imperial Valley’s Equity Community Organizing (ECO) Group, also joined to highlight the critical work happening on the ground in local communities. ? The event underscored an important lesson: meaningful change doesn’t just improve the lives of a few—it creates lasting, inclusive impact for all, especially older adults facing systemic barriers. Insights from disability advocates highlighted how to drive change that creates inclusive opportunities for everyone. ? The SCAN Foundation has long prioritized advancing policy solutions for older adults that are dually eligible for #Medicare and #Medicaid. Through our work, we’ve listened to these people tell us directly that the lack of integrated and coordinated coverage makes it harder than it should be for them to age well in their own homes and communities. That’s why, in partnership with the The Commonwealth Fund and Arnold Ventures, we are proud to once again support Center for Health Care Strategies (CHCS) #MedicareAcademy, which helps build critical Medicare knowledge and capacity amongst state Medicaid agency personnel. Congratulations to Rhode Island and North Carolina on being selected for the upcoming Academy! What if your health care started with your goals and preferences? Personalized care plans that align with individual goals often lead to better health outcomes, but too often, older adults aren’t even asked about their priorities, let alone have clinicians follow up. Enter Person-Centered Outcome (PCO) measures. PCO measures ensure people get health care based on what matters to them, like ensuring a new medication doesn’t cause daytime drowsiness during someone’s daily visit to see their grandchildren or prescribing physical therapy to keep an older runner as active as can be while still addressing their knee pain. These measures help clinicians create care plans that help patients meet their goals and improve their health. By fostering shared decision-making, they empower older adults to take control of their health and strengthen the clinician-patient relationship in the process. Hartford Foundation, our goal is to embed these measures in publicly reported health plan systems, driving quality improvement, accountability, and trust between provider and patient. Through listening projects we support, such as The People Say, older adults make it clear: they prefer to age in place in home and community (bonus: studies show it’s cost-effective, too).? And nearly 1.5 million older adults enrolled in Medicaid receive home- and community-based services (HCBS) that help them age in the places and ways they prefer. This includes help with eating, walking, transportation, and other daily tasks. Not only is this kind of care desired—it’s lifesaving and more financially sustainable as well.
