A Journey to Ending Syphilis in Indian Country The syphilis crisis continues to disproportionately impact Tribal communities. In response, The Raven Collective, in collaboration with NACCHO and Cardea Services, convened three?Regional Syphilis Partnership Summits?in 2024 to develop community-driven solutions using an Indigenous and systems-based approach. What We Learned: ??Partnerships Matter?– Collaboration among Tribal, IHS, state, and local entities is critical for strengthening care coordination and resource sharing. ??Stigma Is a Barrier?– Shame surrounding syphilis, sexual health, and substance use deters people from seeking care. Community Health Workers (CHWs) play a vital role in reducing stigma and increasing trust. ??Capacity Must Be Built?– Workforce shortages and funding limitations hinder effective responses. We need sustainable investments in public health infrastructure and workforce development. The Path Forward: ? Regular cross-sector meetings to foster collaboration and align priorities ? CHW-led outreach and education to normalize sexual health discussions ? Sustainable funding models that prioritize long-term solutions over short-term fixes Ending syphilis in Indian Country requires?community-driven, culturally responsive?strategies that honor Tribal sovereignty and integrate Indigenous knowledge. ?? As an indigenous, female-owned collective, our work is building connections. TRC has 20 years of experience working with American Indian and Alaska Native Health systems. TRC has worked with every element of the health systems involved in HIV, STI and HCV syndemic response: tribal, county, state, and federal. TRC works to improve access and quality has involved many layers, including community-facing educational materials, clinical training, and facility-level policy & practice. TRC has found that each facility and entity has learned wisdom worth sharing, and each has its own constraints. Community, clinical, data, and informatics must interface in many of these health systems that, by necessity, entail multiple health entities. TRC elicits the lessons learned and emerging best practices in these systems, often creating a path for peers in other spaces to achieve similar success.

We’re thrilled to share the?Evaluation Report?for the 2024 Tribal Opioid and Fentanyl Summit: Strengthening Pathways to Prevention, Recovery, and Resilience, held October 8-9, 2024, at the Seven Feathers Casino Resort. This gathering brought together tribal leaders, healthcare providers, public health professionals, and community advocates to tackle the opioid crisis in Tribal communities through innovative, culturally informed strategies. Key Insights from the Summit: ?? Emphasis on culturally relevant approaches, integrating Indigenous knowledge and community wisdom. ?? Diverse participation from tribal health, public health, and justice sectors, including voices with lived experience. ?? Topics ranged from harm reduction and cultural resilience to innovative Tribal justice approaches. We are pleased to announce the release of?We Are Part of Seven Generations: Themes and Recommendations from the Indian Country Congenital Syphilis Taskforce. This initiative highlights critical strategies and policy recommendations needed to address the congenital syphilis crisis impacting Indigenous communities. The Raven Collective, in partnership with Cardea Services and with support and funding from NACCHO, convened the Indian Country Congenital Syphilis Taskforce in January 2024. Last week, a coastal storm caused devastating flooding in Northwest Alaska, displacing an estimated 85 residents just as winter is beginning. The community needs support to rebuild and recover. The Alaska Native Tribal Health Consortium and ANTHC’s Healthy Alaska Natives Foundation (HANF) have partnered to manage a rapid response fund on behalf of Tribes. 100 percent of proceeds will be directed to the Native Village of Kotzebue to help respond to the coastal storm and devastating flooding in northwest Alaska last week. ANTHC and HANF partnered with the Native Village of Kotzebue to manage the Fund so Tribal officials and first responders can focus on recovery. The natural disaster caused the evacuation of an estimated 85 residents from their homes just at the onset of winter. In addition to managing the fund, ANTHC and HANF are donating $20,000 to the recovery effort. Did you know there's a syphilis epidemic in the United States? At least I didn't. But in 2022, we had 200,000 cases of syphilis in the US —?the highest rates since 1950. It's odd because, although chlamydia and gonorrhea rates have been flat or declined in recent years, syphilis has skyrocketed. What's particularly scary is congenital syphilis, which kills two in five babies (if untreated) and leaves many others with permanent disabilities. For NBC News, I wrote a solutions piece about syphilis, covering the most innovative and unconventional ways to curb the spread of disease. It's everything from an Ohio billboard saying "PAYING for SEX? GET TESTED!" to a mobile clinic driving up to homeless camps to testing every pregnant woman who comes into the ER. “The solutions for controlling syphilis are relatively straightforward. We’re not talking about high tech approaches or a novel therapeutic,†said Dave A.
