At Buckeye Hospice Care we are thrilled to be the first Hospice to become a Positive Approach to Care Designated Organization. We love working with Teepa Snow's organization to change the face of dementia care in hospice.
关于我们
Our organization consist of several high performing home health, personal care, and hospice companies within the Midwest. LTM stands for "Living the Mission" which is guided by our philosophy that patients recover more quickly at home where they are the most comfortable. In-home care simply leads to greater independence, reduced hospitalizations, and better outcomes, day-in and day-out. Home healthcare services allow for medical treatments in the most comfortable environment – your home. The beauty of home healthcare is that it can be short or long-term and can change over time. You only get services when and how you need them. Our experience shows that the entire family benefits from home healthcare. Patients and families experience a sense of independence along with dignity for the patient. More often, the caregivers are relieved with having services they can see and patients greatly appreciated not being transported in –and-out of clinics and rehab centers. Everyone benefits from participating in the care and treatment of the patient, at home.

Buckeye Hospice Care Achieves PAC Certification, Leading the Way in Dementia Care Innovation January 27, 2025 – Buckeye Hospice Care is proud to announce that it has become the first hospice provider to achieve Positive Approach to Care (PAC) Designated Organization certification, a groundbreaking milestone in transforming dementia care. This certification highlights Buckeye Hospice Care’s commitment to providing compassionate, person-centered care for individuals experiencing brain change. Incorporating the expertise of renowned dementia care specialist Teepa Snow, Buckeye Hospice Care has adopted the PAC methodology to better serve patients with cognitive and memory challenges. The PAC method focuses on understanding each individual’s unique needs and abilities, emphasizing strengths rather than limitations. This approach ensures that patients receive the highest level of support while honoring their dignity and individuality. “We are thrilled to be the first hospice to achieve PAC Designated Organization certification,” said Jessica Iorio, Hospice Director at Buckeye Hospice Care. “Our team is dedicated to changing the culture of dementia care by implementing Teepa Snow’s proven techniques, which foster a more positive, supportive environment for our patients and their families.” The PAC method provides a shift from traditional care models, encouraging healthcare professionals to focus on what individuals can do rather than what they cannot. This allows for more effective communication, better relationships with patients, and a more fulfilling care experience. With this certification, Buckeye Hospice Care reinforces its role as a leader in innovative hospice and palliative care. The organization is committed to ensuring that all patients receive care that is not only medically proficient but also empathetic, personalized, and deeply respectful. ?? Cold weather alert! ?? With the temperatures dropping, please take a moment to check on your elderly and disabled neighbors. Many of them may struggle with the cold or have limited access to warmth and essentials. A quick visit, a phone call, or offering help with tasks like shoveling snow or bringing in groceries can make a huge difference. Let’s all look out for each other and make sure no one is left vulnerable during these chilly days. Stay safe and warm, everyone! ?? #CheckOnYourNeighbors #WinterSafety #CommunityCare

As I took a day off today and reflected, I wanted to reach out to anyone looking for change. If you’re considering a career change in 2025, look no further than home health. I’ve dedicated my entire career to home care, and it’s stories like these that make what we do every day the best job ever! In home care and hospice, we meet people where they are — caring for them in their homes. It’s not just visiting patients or bringing a smile to their doorstep. We bring comfort, inspiration, peace, independence, and hope. Few careers offer the chance to make such a profound impact. To all the incredible people at The LTM Group and everyone working in home care and hospice Thank you for what you do. Let 2025 be filled with incredible purpose, growth, and continued impact on the lives of those we serve.
