Happy World Mental Health Day! Today, we’re shining a spotlight on the importance of mental health and well-being. It’s a perfect time to reflect on our own mental health and support those around us. ?? Remember: -It is okay to talk about your mental health. -Seek support if you need it – whether through friends, family or professionals. -Self-care matters – take time for activities that bring you joy and relaxation. Last week, The Health Plan team had the privilege of attending Bras Across the Park in Gilmer County! This impactful event, held during Breast Cancer Awareness Month, aimed to raise awareness and provide vital resources about breast cancer. Participants learned about the signs, risk factors, and the importance of screenings and mammograms. Events like this are crucial for fostering a cancer-free community. We're proud to be part of the conversation and encourage everyone to prioritize their health and get checked!

Did you catch our outreach team this September? ?? Our team had a busy month providing resources and information at health and resource fairs as well as dental events all over the state of West Virginia. We also spent time providing meals to those in need with Mountaineer Food Bank. We are so happy to help, educate, and support our fellow West Virginians!

The Health Plan has a variety of clinical programs conducted by professional staff to help you achieve your patient care goals. Health, Wellness and Prevention Programs are offered to provide education, support and resources to members who may require assistance with improving nutrition, achieving a healthy weight, increasing physical activity, managing stress, identifying and addressing depressive symptoms, tobacco cessation or facilitating access to community based organizations to address health determinants (SDOH) such as homelessness, lack of food or transportation. Pregnancy Care Programs are available to both high and low risk members, as well as to members who may want to discuss family planning or birth control options. Chronic Disease Management Programs are available and aimed at helping to educate members and reinforce provider treatment plans for members learning to live with and manage diabetes, coronary artery disease, congestive heart failure, chronic obstructive pulmonary disease with or without comorbid depression. Medical/Behavioral Health Case Management Programs are here to bridge gaps between moderate to high-risk members and providers. Program goals include increasing member understanding of disease or condition, managing/controlling symptoms, adherence to medication regimens, removing any barriers to care and helping members gain control of their health and achieve optimum function.
