"We are now actively working on the implementation of both Chronic Care Management and Remote Patient Monitoring. Thanks to RCHI’s support, we acquired the knowledge needed to make informed decisions and establish a new service that will elevate our patient experience and improve the quality of care we provide. We can’t express how thankful we are for our partnership and the amount of time and resources Texas A&M dedicates to improve rural communities and the health disparities that we work hard to overcome each day." – Limestone Medical Hospital Staff If you want to know more about how we can support your CCM or RPM efforts, reach out to Reyann Davis at [email protected]. #PowerOfRural #RuralHealth #CCM #RPM

In the last 20 years, suicide rates have increased 46% in non-metro compared to 27% in metro areas. Healthcare providers in rural areas are in a unique position to save a life: one-third of individuals who die by suicide visited primary care in the month before their death. If you want to learn how to prevent a tragedy in your community, check out the SafeSide Framework for Suicide Prevention in Rural Communities online workshop on August 15th. Seats are limited for this excellent free training, so register today! https://lnkd.in/eH--Kc2S
