Earlier this year, Telligen and CPESN were awarded the Civitas Health Equity Champion Award for efforts to increase vaccination rates at over 100 nursing homes and provide education at more than 1,000 facilities across the country. In addition, over 5,000 beneficiaries received personalized vaccine screening and education at CPESN pharmacies through this partnership. This award recognizes a single organization that has brought community leaders across sectors and stakeholders together to identify root causes of inequity, developed a plan for addressing those root causes, and identified a sustainability model that incentivizes and motivates the advancement of health equity.
