Program of All-Inclusive Care for the Elderly **The PACE approach** Your care is provided by an Interdisciplinary Team (IDT) that is assigned to each participant consisting of the following staff: ? Physician ? Registered Nurse ? Home Care Coordinator ? Masters level Social Worker ? Occupational Therapist ? Physical Therapist ? Recreational Therapist ? Registered Dietitian ? PACE Center Manager ? Certified Nurse Assistant ? Transportation Driver The IDT assesses each participant’s needs and delivers services based on a customized care plan.
