The Hill-Burton program, formally known as the Hospital Survey and Construction Act, was a pivotal turning point in the advancement of rural healthcare in the United States. Signed into law in 1946, this legislation transformed the landscape of rural health by providing crucial construction grants and loans for building healthcare facilities in communities that previously lacked them. Enabling the construction of nearly one-third of U.S. hospitals by 1975, the Hill-Burton program brought essential medical services to the doorsteps of many rural communities, enhancing healthcare accessibility significantly. Even after its direct funding ended, the legacy of the Hill-Burton program continued. Now, with over 1300 Critical Access Hospitals currently active in the United States, many have reached the turning point for them to be replaced, expanded, or updated in order to continue serving their rural communities. In 2022 Stroudwater Capital Partners - Rural Healthcare and Stroudwater Associates released the 2022 Rural Hospital Replacement Facility Study considering data from all Critical Access Hospitals replaced between 2000-2019 and in fall of 2025 the updated study will be released extending the data from 2000-2023. Outmigration occurs when patients from rural areas travel to larger hospitals in more populated areas for healthcare services. This phenomenon can have significant impacts on rural healthcare systems such as affecting patient volume and market share for rural healthcare facilities, financial strain, damage to the reputation of local healthcare providers, among others. In 2022 Stroudwater Capital Partners - Rural Healthcare and Stroudwater Associates released the 2022 Rural Hospital Replacement Facility Study considering data from all Critical Access Hospitals replaced between 2000-2019. The findings relative to volume growth post-replacement and the corresponding decrease in outmigration for Critical Access Hospitals was promising. Much has been written and/or studied about the attributes of an effective #salesperson or #consultant. Be a good listener and empathetic. Be a thoughtful and an intuitive problem solver. Be adaptable and resilient. For a brief time early in my career, I was a medical staffing consultant with The Delta Companies. Scott Hurst MBA, FACHE, Jim Fuller, and Thomas Riddle hired me to work with hospital leaders on developing strategies to address physician shortages. At least, that’s how I saw it. I was a young and hungry newlywed who was eager to build a career and provide for my family. My “territory” was northern California, Oregon, and Nevada, a long jaunt from my then home in Dallas. I didn’t mind the travel (especially before kids) and honestly believed I was making a difference for my clients; most were in rural and underserved areas. Southwest Airlines is a fine carrier, but sitting in a seat (that reclines an inch) late into an evening will wear on most folks over time. At some point, the glamour of collecting points is replaced by the reality of Starbucks and packaged pretzels for dinner, singing to my kids over the phone, and falling asleep alone in Marriott Hotels. Looking back, I remember the excitement of working with #hospital leaders who validated my passion and expertise. I wanted to work extra hard for them, ensuring they were successful. I also remember flying to hospitals where the leader “no-showed,” cancelled at the last minute without explanation, or were physically present, but clearly distracted. I wondered in those moments if I cared more about their outcome than they did. As I transitioned into a career as a hospital CEO, I found myself on the receiving end of these visits. Admittedly, my own experiences faded and the “whirlwind,” as Chris McChesney calls it, tested my ability to be present. There must have been times when sales folks left my office disappointed, not by my decision, but rather by my distraction. Some of you reading this post may have experienced me this way. If so, I’m sorry. Some brief work with Stroudwater Associates several month ago refocused my perspective. Stroudwater sets the standard for value and preparedness in the #healthcare consulting industry. I don’t know a brighter finance mind than Eric Shell, and Opal (Hammer) Greenway is preeminent in physician compensation. Here is my challenge to my LinkedIn family: Like Eric and Opal, many of the folks who travel to meet with you are making sacrifices, hoping provide value to you. When they do, be a good listener and empathetic. Be a thoughtful and work with them to solve problems. Take interest in them as people. As you bring out the best in them, they’ll bring out the best in you.
