Engagement is key to managing the care of patients with chronic conditions, like diabetes, heart disease or obesity. It is for this reason that Stability Health has developed a successful care model that combines innovative technology with a coaching platform. Its technology platform contains a registry driven by a rules engine. The rules engine has embedded evidence-based standards of care from a variety of professional organizations, such as American Diabetes Association, American Heart Association, Association of Certified Diabetes Care and Education Specialists, and is able to ingest data from a variety of devices. Patients are then monitored when they are in their usual routines. The platform integrates Information collected through detailed patient assessment, driven by the rules engine, to create a comprehensive care plan. This plan supports patients in making lifestyle changes, and delivers specialist expertise to their clinical care teams, so that they make optimal, clinical decisions for medications and devices. In addition to the technology, Stability Health’s transformative care model supports patients with a human coach they connect with virtually using their preferred technology - phone, e-mail, text, video chat. Patients also have access to specialists to support more intensive education when needed, or requested - occurring at a time that is convenient for them. Medical specialists oversee and ensure care plans, and recommendations are appropriate for each patient. In this way, the entire care team, supported by the platform that standardizes ideal care, provides individualized care to each patient.

At Stability Health, we stay up to date on the latest recommendations for #diabetes care and bring this knowledge to our clients.?The American Diabetes Association has come out with the 2025 Standards of Care.?The section on #DiabetesCare in Older Adults has been re-written to align with Institute for #HealthcareImprovement (IHI) “4M” framework for age-friendly medical care.?We have already revised our care plans and guidance to our client frontline clinical teams.?It is all about individualizing goals of diabetes care for this population. #healthcare #medicalcare

While I didn’t attend JPM this year, I’ve been seeing a lot of content about what was presented.?One thing that caught my eye was a summary about a different approach to Medicare Advantage, a strategy to treat it like a care management business, instead of an actuarial valuation business. This really resonates with me.?I’ve spent the last 10+ years immersed in value-based care.?My experience around Medicare Advantage is that it’s been driven by a revenue model.?I get it. It’s important to get risk coding right. It’s what drives the business engine for the plans and groups that take the risk. However, as a clinician, this approach always felt unsatisfactory.?When the strategy is not about how many more conditions you can mine from the chart, then the opportunity becomes how to align payers, care teams, and patients in an improved system of care.
