Southwest Virginia Community Health Systems: Opening Doors to Quality Healthcare Organizational Profile: SVCHS consists of a group of non-profit Community Healthcare Centers, a Migrant Health Network, an Integrative Behavioral Healthcare Program, a Substance Abuse Recovery Program, and the Southwest Virginia Regional Dental Center. SVCHS was established to improve access to healthcare services by providing comprehensive, patient-centered care to the communities we serve. Mission Statement: SVCHS is a community healthcare partnership serving the people of this region with compassionate, affordable, patient-centered primary and comprehensive care. Vision Statement: SVCHS, Inc. will provide compassionate, patient-centered, affordable healthcare to all patients, nationally recognized for its excellence in all aspects of care. About: SVCHS is a member of the Virginia Community Healthcare Association and is a Joint Commission-accredited institution. SVCHS is governed by a community board, and our clinics are not-for-profit Community Health Centers. There are almost 3,500 similar centers nationwide, with 65 serving Virginia. SVCHS is a fee-for-service practice just like any other doctor’s office. To assist the center in providing access to medical, behavioral health, and dental care to all residents regardless of income, SVCHS receives an annual grant from the U.S. Public Health Service. Being a community-based organization, we extend an invitation to you and your family to use our facility. We also welcome any input and comments on how we can better serve you.

The Human Resources Director is responsible for Human Resources and credentialing, privileging, and recruitment. The specific duties of the Human Resources Director are defined in the following detailed position description. Specific duties and responsibilities include, but are not limited to the following: ? Responsible for SVCHS compliance with federal and state legislation about personnel matters. ? Coordinates orientation, annual evaluations, and competency assessments for all staff. Calculates increases based on scores. ? Assists accountant with payroll as needed. ? Data entry to set up and maintain payroll records, timekeeping, training module, and personnel files. ? Responsible for interviewing and hiring administrative, clerical, and custodial staff. ? Assists with recruitment and retention of provider staffing. ? Provide orientation to all new staff. ? Conduct exit interviews to determine reasons for separation. ? Maintain Human Resources certification through continuing education. ? Represent SVCHS on community partner boards and committees. ? Assists with grants and reports about personnel and human resources. ? Supervision of credentialing analyst and medical records clerk. ? Provide training/education for new staff and annually for all staff to maintain awareness of personnel laws about sexual harassment prevention, ADA, violence in the workplace, FMLA, etc. ? Coordinate annual employee satisfaction surveys and disseminate responses to all staff. ? Strive to develop and maintain new processes that meet SVCHS personnel needs. ? Serves alongside CMO/Medical Director and Medical Operations Director as part of the risk management team. ? Provide conflict resolution, as required. ? Serves as claims manager for all actual or potential claims including malpractice and commercial. Qualifications include: ? Graduation from a business or technical school or equivalent education/certification. ? A minimum of five (5) years experience in Human Resources. ? Two (2) years of supervisory experience. ?   • WE'RE HIRING - Clinical Pharmacist - Meadowview Southwest Virginia Community Health Systems (SVCHS) is seeking a full-time Clinical Pharmacist to join our healthcare team. The Clinical Pharmacist will provide ambulatory clinical pharmacy services at multiple sites within the organization. The Clinical Pharmacist will: ? Identify and resolve medication-related problems ? Work collaboratively with medical staff to manage and improve health outcomes of patients with chronic diseases ? Interview patients to obtain information regarding medication use, medication allergies/sensitivities and document the information in the patient’s medical record ? Develop, assess, evaluate, and plan drug therapy regimens ? Recommend medications, including initiation, continuation, discontinuation, and alternate therapies ? Assist SVCHS staff to ensure patients are able to obtain medications in the most cost effective manner possible to improve medication adherence ? Provide patient education regarding medications, medication devices, and chronic disease states ? Serve as a drug information resource for medical staff ? Assist in completion and submission of medication Prior Authorizations ? Participate in the creation and/or review of protocols involving medication use ? Perform medication utilization evaluations for high risk medications ? Provide pharmacist staffing coverage for the entity-owned retail pharmacy ? Maintain knowledge of the 340B Drug Pricing Program The qualified applicant will have a Doctor of Pharmacy degree from an accredited school of pharmacy. Completion of one year of residency and/or board certification in Ambulatory Care (BCACP) or Pharmacotherapy (BCPS) is strongly preferred. CDCES or similar credential would be a bonus! A valid pharmacist license in the state of Virginia, or ability to obtain within 60 days, is required.
