We recently added a new portfolio company with an investment in Sonara Health. Sonara is a digital platform that helps people with opioid use disorder (OUD) follow their methadone treatment program easier and more compassionately. Getting methadone can be difficult—and often a barrier for people in recovery, particularly during the critical first 90 days of treatment. Over 9 million Americans need medications for OUD, yet only 25% receive the treatment they need. Traditional treatment centers and methadone clinics require patients to come in person every day, where they often face long wait lines. These geographic and logistical requirements create unnecessary hurdles for people in recovery. Sonara’s platform addresses these barriers by providing treatment facilities with a web-based application for remotely observed dosing. Rather than traveling to the clinic each day, patients can record their dose administration remotely (for example, via secure video). This real-time insight into adherence allows clinicians to build trust faster than they could through in-person visits alone, leading to earlier eligibility for take-home doses and, ultimately, fully unmonitored dosing. This accelerated path to autonomy, coupled with fewer clinic visits (often just once a month), dramatically reduces the daily disruptions that can derail recovery. Since its launch at the end of 2022, Sonara has supported more than 1,000 patients across 50 opioid treatment programs in 12 states. By enabling a quicker, more convenient way to establish trust and independence in treatment, Sonara is dismantling the obstacles that prevent so many people from accessing—and staying on—life-changing OUD care. With Sonara’s Virtual Dosing Window?, eligible patients can now record their take-home methadone doses in the Sonara app—helping them earn take-homes faster, reduce clinic visits, and gain more flexibility in their recovery journey. We’re proud to partner with Pinnacle Treatment Centers to expand access to patient-centered care and support individuals with Opioid Use Disorder.

For many, the dream of recovery—holding a job, taking their kids to school, or pursuing an education—is put on hold due to barriers that limit access to take-home methadone. Last week, our VP of Operations, Sheeba Ibidunni, and Chief Operating Officer, Justin Geller, testified before the Ohio House Children and Human Services Committee, advocating for an amendment to House Bill 96. This amendment would fund a statewide pilot program for remote observation of take-home methadone, a solution that expands access while ensuring safe and effective treatment. With Sonara, take-home methadone becomes more than just a possibility; it becomes a pathway for people to reclaim their lives and pursue the futures they’ve dreamed of.

SAMHSA encourages providers treating opioid use disorder to implement low barrier care models, but what does that actually look like in practice? Key principles of an effective, low barrier care model for OTPs include: ?? Patient-centered care: Empower patient decision-making and collaborate with patients to build personalized treatment plans. For OTPs, this can look like basing visit frequency on clinical stability, considering comorbidities when building a treatment plan, and not restricting treatment based on self-reported drug use. ?? Flexibility and barrier reduction: Adapt to individual's specific needs and circumstances by offering flexible treatment options. Employing a remote dosing solution like Sonara Health allows OTPs to safely and confidently issue take-homes. ?? Comprehensive and inclusive care: Consider patients' whole health, supporting medical, behavioral, and social needs. For example, consider connecting patients to services that support recovery outside of the clinic. ?? Commitment to growth and continuous learning: Commitment to organizational improvement, providing ongoing opportunities for growth and education. This may include training on the latest evidence-based interventions, trauma informed care, education about harm reduction, and cultural competence training.
