An innovative Adult Social Care transformation programme will save Somerset Council over ￡10 million by the end of March 2025 while also improving outcomes for residents. Stacey’s (not her real name) story is a touching example of personalised care and community support. Living with autism and generalised anxiety disorder, Stacey has become quite isolated, spending most of her time at home near Mendip. Despite her isolation, she has a keen interest in the outside world, particularly in politics, philosophy, and religion. Recognising her potential, Stacey’s social worker proposed that she could benefit from having someone visit her to help build her confidence in becoming more engaged with her community. To ensure these hours were used effectively, the social worker sought advice from a peer forum. This collaborative approach led to the suggestion of involving a community agent to explore local activities that might interest Stacey. By identifying specific activities and dedicating around 5 care hours a week to support her participation, the plan aims to help Stacey achieve greater independence. This thoughtful and tailored approach considers Stacey’s unique needs and interests, providing her with the opportunity to engage more with the world around her and enhance her quality of life. This story highlights the quiet but important work of adult social care that happens every day, making a significant difference in the lives of individuals like Stacey. ??
