To help families and professionals stay on top of the latest terms, we're going to share (and define) some common phrases we get asked about. We hope it helps! Up next: Person-Centered Planning Tool (PCPT) This mandatory discovery document is used to guide the development of the #ISP. It lists an individual’s family members, friends, and guardians, details regarding how an individual views him or herself, employment details, relevant career experiences, mental health information, and more. What is an Individualized Service Plan (ISP)? ?? Your support coordinator will work with you and your loved one to develop an ISP, a customized planning document required by the #NJDDD. It will outline your loved one's goals and steps to achieve them, ensuring they have access to the support they need to remain healthy, safe and a valued member of their community. Their plan will typically include information about personal preferences, medical history and concerns, communication preferences, along with dreams and aspirations.
