Geriatrics primary medical care at home or place of residence. Senior Care Clinic House Calls' mission is to provide personalized, high-quality care on an as-needed or ongoing preventative basis in the home environment where our seniors live. We do geriatrics primary care service with a dedicated Nurse Practitioner (NP), in collaboration with our Medical Director. We believe in working with our clients to maintain their level of function in the setting of their own home or place of residence (e.g. Retirement Home, Independent Living, Assisted Living, or Board & Care homes). We manage chronic medical conditions such as hypertension, diabetes, wound, dementia, palliative care,and the like. We also believe in sharing knowledge and decision making with our clients, and their designated responsible persons, as we work with them to achieve the best physical state possible at their current level of function. We always consult with the family or responsible person in order to provide informed, consistent care at all times.

*Blair Findlay, HCCI Development Associate, rode along with Dr. Paul Chiang, HCCI’s Senior Medical and Practice Advisor, for a day of house calls. Findlay shares her experiences and insights from that day. At HCCI, we believe that the future of healthcare is in the home. What do you see when you picture “home”? The four patients I saw with Dr. Chiang have very different homes—Betty and Gary lived alone, Quang lived in a multi-generational home, Fazli and Ilma lived in an apartment, and Elsa lived in a nursing home that cost upwards of $6,000 a month. These patients had different racial and economic backgrounds, but by having access to house calls, they had equality in the quality of medical care they received. House calls are for everyone. Quality medical care isn’t limited to one’s location or socioeconomic background. When providers come to HCCI to learn how to offer home-based primary care, they learn how to connect and show care for these homebound and home-limited patients.
