Over the last 18 months at?Sama Therapeutics, our team has designed, built, and tested medical AI/ML incorporating every single bullet point reviewed here (and more besides) into our codebase — creating an unprecedented, scalable R&D platform product to enhance precision clinical trial analytics and healthcare provider workflows in neuropsychiatry. To control for overfitting, data leakage, and stochasticity, we challenged the multimodality, meta-learning model’s treatment outcome prediction performance across 4 different diagnoses (MDD, ADHD, OCD, and PTSD) and 3 different interventions (TMS, neurofeedback, and SSRIs), achieving between 125-150% nPPV, ie positive predictive power normalized for imbalances in base responder rates between cohorts. We further verified our approach in the most rigorous possible fashion, via 3rd parties who conducted blinded, out-of-sample replications — winning Sama Therapeutics’ “transprognostic” algorithm the top 4 positions on the BrainClinics Foundation’s international leaderboard to date?https://lnkd.in/exTw3wQn?and publications forthcoming. A twelve item checklist for clinicians to critically evaluate and design better clinical machine learning studies: 1?? Assure data is of sufficient quality to build relevant clinical projects. 2?? Establish a baseline performance standard. 3?? Examine the complete machine learning workflow and avoid misleading metrics. 4?? Ensure classes are balanced, and if not, use precision and recall instead of accuracy and error rate. 5?? Gauge performance differences between models using statistical techniques. 6?? Confirm the model is robust to random factors before reporting performance metrics. 7?? Ascertain model performance by using training, validation, and testing datasets. 8?? Determine overfitting by tracking training, validation, and test set performance metrics. 9?? Check for bias will ensure future machine learning models promote more equitable healthcare. ??
