Our facility and other local community leaders spent the day identifying gaps and barriers to mental health and collaborated on specific and measurable solutions. The College of Coastal Georgia held its “Walk a Mile in Her Shoes” event last week to bring awareness and resources on domestic violence. We were honored to be a part of this event. This year’s Veterans Health Administration (VHA) National Community Partnership Challenge theme, "Bridging the Gap Through Partnerships," celebrates and highlights how the VHA and community organizations develop non-monetary partnerships to meet Veterans’ needs with creativity and innovation. This theme seeks to recognize community partnerships that are built to meet healthcare needs by identifying gaps in services or resources across social determinants of health. Social determinants of health are conditions in the environments in which people live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks in the categories of education, employment, food security, housing, spiritual support, and transportation. Last month, we had the honor of receiving an award from the VA regarding our ongoing collaboration and community partnership. This partnership comprises five external community partnerships: Wekiva Springs Center, University of Florida, St. Simon By-The-Sea, Georgia Pines Community MH/DD/AD Services, and Archbold Northside, working collaboratively with the VA Suicide Prevention team to reduce the number of Veterans dying by suicide. The partners developed a referral process that notifies the VA Suicide Prevention Coordinator when a Veteran presents for either suicide ideation or a suicide attempt at a community-based partner. The Suicide Prevention Coordinator meets with the Veteran at the community partner’s facility and connects the Veteran to care before leaving the facility. We are so honored to receive this recognition.
