Our annual Breast Cancer Walk was held yesterday to support the cause and bring awareness to the disease that impacts so many. Sage Outpatient wishes to support those battling breast cancer and to encourage everyone to perform self breast exams and participate in mammograms as recommended by your physician. “We are so honored that we’ve been selected to participate in the CMS GUIDE project and to be able to offer enhanced services for our dementia patients throughout our network.” Said Area VP of Palliative and Aging Care Shannon Wentz. “We know that early intervention is key to improving the quality of life for both dementia patients as well as their caregivers and family members.” Launched on July 1, 2024, the GUIDE Model will test a new payment approach for key supportive services furnished to people living with dementia, including comprehensive, person-centered assessments and care plans; care coordination; 24/7 access to an interdisciplinary care team member or helpline; and certain respite services to support caregivers. People with dementia and their caregivers will have the assistance and support of a Care Navigator to help them access clinical and non-clinical services such as meals and transportation through community-based organizations. #GUIDE #CMS #Medicare #Medicaid #healthcare #dementia #Partnership
