Families also need support, as they are often supporting their family member with Down syndrome. Support is a key human right that helps make other rights possible. SacValley MedShare has partnered with the California WIC Association to enhance the management of WIC referrals through our health information exchange (HIE). This groundbreaking collaboration is set to improve outcomes for women and children throughout Northern California. California WIC association provides free healthy foods, breastfeeding support, nutrition education and referrals to health and community resources for your family. With this program, we aim to: Streamline the WIC referral process using advanced health information exchange technology. Increase enrollment among eligible participants. Develop a scalable model that can be replicated in other regions. We’re excited to see how this initiative will automate workflows, enable real-time data sharing, and improve care coordination. These advancements will help ensure timely access to critical WIC services and lead to better health outcomes.
