Perforated cholecystitis is not always a contraindication to percutaneous cholecystostomy. In this case, the patient was septic, anticoagulated and had multiple comorbidities that precluded a trip to the OR. The coronal reconstructed image demonstrates discontinuous appearance fo the caudal gallbladder wall with adjacent fluid indicating perforation. After prescanning and identifying an intercostal site of access (first axial image), I used trochar technique to avoid potential seeding of the catheter tract and placed a locking 10F drain (2nd axial image)to aspirate 140cc of bile and debris (final image). Thanks to our radiology department nurses and CT technologists, we had the patient in and out of the CT room in 25 minutes with procedure/sedation time of 20 minutes. #interventionalradiology LucidHealth Riverside Radiology & Interventional Associates OhioHealth
