What We’ve Learned About Medical Documentation (And How We’re Evolving Stream) At River Records, we started with a simple but powerful idea: clinical information should be organized by medical problem or topic. Why? Because the electronic health record (EHR) is not designed for clinical care—it’s designed for billing. Most clinicians agree that today’s EHRs make it frustrating to review patient histories, forcing them to hunt through bloated, scattered, or outdated documentation just to find the information they need. We believed that organizing documentation by problem would make chart review and follow-up easier. And while clinicians loved this idea in theory, in practice, it disrupted workflows too much—some even told us it “broke” their brains. So we iterated. We first made it easier to generate a traditional SOAP note from a problem-oriented format, giving clinicians a way to merge problem-based documentation into the structure they were used to. But that still wasn’t enough. Now, our latest version of Stream supports all major documentation styles—because the reality is, documentation needs to work for clinicians today, not just in the future.
