November's Image of the Month features Retinal Detachment with a Giant Retinal Tear. This beautiful image shows the superior retina falling over the macula because of a giant retinal tear. Giant retinal tears are defined as a full-thickness retinal break extending circumferentially for 3 or more clock hours. Tears of this size tend to lead to rapid retinal detachment and highly mobile and unstable retinas. Hypotony and proliferative vitreoretinopathy are common due to the large area of exposed uvea. Vitrectomy repairs commonly utilize techniques involving perfluorocarbon liquids, which are heavier than water and can be used to unfold and stabilize the retina intraoperatively to allow for adequate retinopexy. Silicone oil tamponade is commonly used in the surgical management of giant retinal tears.
