Despite advances in alignment strategies, surgical techniques, and implant design,?many patients remain dissatisfied with their total knee arthroplasty (TKA)—and a new study helps explain why. ? Researchers analyzed?85,604 preoperative CT scans?to compare real-world knee morphology to 12 common off-the-shelf (OTS) implants. Their findings??Less than half (41%) of patients had a proper implant fit, and nearly?40% of knees?would require significant compromise in implant positioning due to variations in distal condylar offset. ? ???Key takeaways: ??Knee anatomy is highly variable—with significant differences based on sex and deformity. ??Most OTS implants are symmetric, despite?consistent asymmetry?in femoral geometry. ??Single-radius femoral designs may not suit all patients, given differences in posterior and distal condylar offset. ?
