Lessons from the Bering Sea - Accountability over Blame "During a Coast Guard patrol in the Bering Sea in 1992, our boarding party faced a critical task: inspecting the Alaskan fishing fleet and preventing illegal fishing by other nations. On a calm day, a Seaman (E3) in our boarding party accidentally dropped his M-16 while climbing a ladder from the rigid hull inflatable (RHI) to the ship we were boarding. The weapon bounced off the RHI and sank into the sea. To make matters worse, the captain and senior officers were monitoring the boarding in real time, leaving us all in disbelief and bracing for severe consequences. The Cutter was 200 yards off the starboard bow. After the inspection concluded, the ride back to the ship felt excruciatingly long. As soon as the Rigid Hull Inflatable (RHI) was lifted and secured from the water, the shipboard public address system (1MC) crackled to life, requesting that the entire team report to the bridge—not just the individual who lost the rifle. Our captain’s response, however, was both unexpected and profound. Rather than singling out the individual, he emphasized team accountability. He insisted that we, the boarding party, take ownership of the loss, learn from it, and move forward together. In his view, responsibility for the failure was shared by the entire team. The captain’s primary concern was our safety - not the lost weapon - reinforcing an essential leadership principle: people’s well-being always comes first. Supplies can be replaced; lives cannot. This incident became a powerful lesson in leadership, showing how prioritizing safety and fostering a culture of collective responsibility can transform failures into valuable learning experiences. What you’ll do: Conduct on-site visits to mentor employees and subcontractors on health, safety, and environmental compliance. Develop corrective action strategies and provide expert safety guidance to field teams. Analyze and report SSEDD metrics and trends to senior management.
