Nutrition is a core pillar of kidney disease management. Here in the U.S., we talk about barriers like awareness, education, buy-in, insurance, utilization, even provider perception and influence. Globally, we see many of these obstacles made even more difficult by the fact dietary counseling simply isn't available in two-thirds of low and low-middle income countries. In fact, it's never available in nearly a quarter of low-income countries. Earlier today we put out our audio recap of the Expert Q&A on kidney nutrition. It's an easy 20-minute listen to review what we learned in the longer-form interview that went out last year. We asked five renal dietitian experts to weigh in on progress, pitfalls, and potential in the kidney nutrition space. It's excellent. The availability of dietitians or renal dietitians for nutritional care of non-dialyzed CKD and chronic dialysis patients varies enormously according to the patients’ income status. The availability of renal dietitians for CKD and chronic dialysis patients is less than 10% for low-income patients, about 17–20% for lower-middle income patients, about 39–54% for upper-middle income patients, and about 72–77% for high income patients. ?????????? ??????????????????: ? Only 48% of the 155 countries have dietitians/renal dietitians to provide a specialized kidney nutrition service. ? 41% of the countries DID NOT provide formal assessment of nutrition status for kidney nutrition care. ? Dietitians and nephrologists only communicated “sometimes” on kidney nutrition care in ≥60% of countries globally. ? Dietary counseling, provided by a person trained in nutrition, was generally not available in 65% of low-/lower middle–income countries and “never” available in 23% of low-income countries. ? The availability of oral nutrition supplements varied globally and, mostly, were not freely available in low-/lower middle–income countries for both inpatient and outpatient settings. And to be clear, it's not only an income challenge. Here in the U.S., take medical nutrition therapy (MNT) as an example. It has been available as a benefit since 2002. From USRDS data, utilization remained <10% for many years. It has slowly increased, but still remains sub-20%. *** What do you think people should know or understand about effective kidney nutrition management (and access) that they may not know already?
