Rely Health provides you with the infrastructure to design and deploy EHR-Integrated Care Navigation program(s) within 4 weeks for any hospital or health plan use-case. We recruit, train, and staff local Patient Care Navigators that can serve and in-person or remote liaisons to drive patient care. A technology suite is used to design modern digital pathways for your health system / health plan. Patients or members never have to download an app or set a password; all engagement is delivered through modern native channels (SMS-Text, Email, Phone Call). Patient Care Navigators have access to a Patient Relationship Manager (proprietary software) that seamlessly integrates with any EHR to drive digital journeys, manage task queues, and facilitate follow-up actions such as automated appointment reminders. Our Care Navigation programs have helped health systems / health plans fill in quality-based gaps (i.e., 53% decrease in ER visits for high utilizers), and drive better acquisition, retention, and satisfaction metrics (i.e., 60% conversion of patients with no PCP to an in-network PCP) amongst their populations.

David reflects on his time as a navigator with Rely Health and how his upbringing and background has formed the way he treats the patients he serves. David writes, "After nearly a year at Arrowhead Regional Medical Center's ER, I've gained invaluable perspectives on patient care. One simple, yet profound, observation: Many patients need clean socks. This reminded me of an old military joke: the cure for any ailment is changing your socks, hydrating, and maybe some Motrin. But there's truth in this humor – small gestures can have a big impact. Let's expand our view of Harm Reduction to include these 'small gesture, big impact' moments. A clean pair of socks might seem trivial, but it can make a world of difference to someone in need. My father's life was changed by a stranger's kindness – a simple burrito offered when he was a hungry orphan. That small act shaped his entire outlook on life and generosity, which in turn has shaped mine. In our ER, we may not be able to shield patients from all harm, but we can offer clean socks. It's a small way to show patients they matter and that someone cares. Remember, we never know how our small acts of kindness might profoundly impact someone's life. Let's continue to make a difference, one pair of socks at a time. ????" We are looking forward to attending and sponsoring the Hospital Association of Oregon Annual Member Meeting next week. We have produced impactful results in supporting #ValueBasedCare, #Hospital Discharge programs, and closing #caregaps with some of our partners in Oregon (Vituity, CHI Mercy Health, Umpqua Health). This will be a great opportunity to present some of our programs results and continue to build relationships with #healthcare leaders in Oregon.
At Rely Health, education is a critical component of our mission to simplify healthcare and build healthier communities. Our care navigators don't just guide patients through complex systems - they empower individuals with life-saving knowledge. A prime example is our work distributing Narcan and educating the public on its use. By putting this overdose-reversing medication directly into people's hands and teaching them how to administer it, we're quite literally saving lives. Our navigators take the time to explain: ? How to recognize signs of an opioid overdose ? Proper Narcan administration techniques?? ? The importance of calling 911 even after giving Narcan ? Our team of navigators had the privilege of learning from Dr. Lorenzo Leggio, a leading expert in addiction medicine. As the Clinical Director, Deputy Scientific Director, and Chief of the Translational Addiction Medicine Branch at the The National Institute on Drug Abuse (NIDA) and National Institute on Alcohol Abuse and Alcoholism, Dr. Leggio shared invaluable insights on the science of alcohol use disorder (AUD) and addiction, as well as recent advancements in Medication-Assisted Treatment (MAT) options. This knowledge is crucial given the substantial economic burden of alcohol and substance use in the United States. With annual costs reaching $249 billion and alcohol-related disorders accounting for nearly $10 billion in medical expenses for employer-sponsored insurance plans, the impact is staggering. Patient navigation emerges as a promising solution to address these challenges. By enhancing treatment engagement, minimizing hospital readmissions, and fostering long-term recovery, patient navigation has the potential to significantly reduce healthcare costs associated with alcohol and substance use disorders. https://lnkd.in/eQzzathe https://lnkd.in/g_CGWCFn

Great work here by Montefiore Health System. More often than not issues that #hospitals are trying to address, ie. #ED throughput, #LOS, #Readmissionrates, have causes that need to be addressed outside of the #hospital walls. We are often asked if #carenavigation can effect these hospital performance measures and the first thing we look at is #communityresources,#SDOH, and #accesstocare. Successful programs require establishing and maintaining partnerships with community resources (Bridge, Plymouth Housing, Lyft health), #PCP/ #UC #telemedicine providers (Vituity & Jovive Health), and many times #payors (Umpqua Health, IEHP,Molina Healthcare) to connect all of the dots in the #healthcare journey and ensure all discharge orders can be followed. We recently had the pleasure of consulting with Swati Mehta MD, FACP, SFHM, CPXP and Wade Fox at Vituity to design a full end to end #Px and #CareNavigation program for a rural hospital to address Patient Experience in the ED. In our evaluation we found that this rural communities lack of PCP and UC access was causing increased ED volumes, throughput issues, and increased readmission rates and realized that telePC and teleUC would be necessary to provide the appropriate level of care settings to address the cause of the issues. So YES, Px/Care Navigation programs can help positively impact 'in hospital' metrics that you are seeking to improve, but this is not a 'point solution' and connecting the hospital to the community it serves to ensure access and continuity of care is essential for program success.
