Rely Health provides you with the infrastructure to design and deploy EHR-Integrated Care Navigation program(s) within 4 weeks for any hospital or health plan use-case. We recruit, train, and staff local Patient Care Navigators that can serve and in-person or remote liaisons to drive patient care. A technology suite is used to design modern digital pathways for your health system / health plan. Patients or members never have to download an app or set a password; all engagement is delivered through modern native channels (SMS-Text, Email, Phone Call). Patient Care Navigators have access to a Patient Relationship Manager (proprietary software) that seamlessly integrates with any EHR to drive digital journeys, manage task queues, and facilitate follow-up actions such as automated appointment reminders. Our Care Navigation programs have helped health systems / health plans fill in quality-based gaps (i.e., 53% decrease in ER visits for high utilizers), and drive better acquisition, retention, and satisfaction metrics (i.e., 60% conversion of patients with no PCP to an in-network PCP) amongst their populations.

Many applications of AI in healthcare are years away from realizing their true potential. Not because the capabilities of the tech are inadequate. Rather, customers are not prepared for its integration in to existing workflows, technology stacks, etc. Agentic AI is NOT one them. Its time is now. Work force/labor challenges. Increased demand for personalized care. Compressed hospital margins. These and many other factors are driving rapid adoption of this technology. Estimates are that this market will grow at 35-40% over the next 5 years. However, much debate still exists around how this Agentic technology should best be deployed. Is the technology ready to standalone? Or is a "co-pilot" model more appropriate, where the AI technology is augmented by human capital to ensure accuracy and efficiency?
