I am so happy to share the news that our team in the @UMCUtrecht and St. Antonius Ziekenhuis Ziekenhuis and Diakonessenhuis received a grant from the Dutch ZonMw, to improve palliative care for cardiac patients. The project is called UPSTREAM-HF. It is actually complementing the RAPHAEL Horizon Europe project, so a great future ahead in the area. New statement out on palliative care for people with heart failure. Members of the RAPHAEL Horizon Europe project Philip Larkin and Hullin Roger have been involved in the new statement from the national Swiss Societies of Cardiology and Palliative Care on palliative care for heart failure patients. ? The paper led by Dr. Piotr Sobanski (National Strategy for Integrating Palliative Care into Standard Cardiac Care for People Living with Heart Failure) is a Position Statement from the Joint Working Group of the Swiss Societies of Cardiology and Palliative Care. The RAPHAEL Horizon Europe project integrating palliative care into heart failure care welcomes the publication of the consensus statement from Heart Failure Society of America. It addresses the need for comprehensive and equitable palliative care to be consistently available to heart failure patients across the spectrum of their illness. And concludes ''Institutions and their HF programs must reaffirm their commitment to meeting the evolving needs of patients and their caregivers, which includes the increasing demand for holistic, patient-centered care that incorporates principles of PC.'' #heartfailure #palliativecare

The ACNAP 2025 congress is organised by the Association of Cardiovascular Nursing and Allied Professions (ACNAP). The theme of this years congress is : Nurses and Allied Professions Leading the Way: Transforming Healthcare for a Sustainable Future.
