Prostate radiotherapy treatments can be fully automated, from acquisition of the CT to a final treatment plan. Mark Gooding et al. summarizes this well in a recent Green Journal paper (link below). This goes hand in hand with an excellent presentation from Jakob Liermann at the RayStation User meeting hosted at RaySearch Laboratories last week, showcasing the clinical implementation of a fully automated workflow for prostate treatments at the University Hospital Heidelberg (UKHD). The radiation oncologist is readily presented with a deep learning (DL) treatment plan based on a DL segmented CT. Even if some manual adjustments are needed, there is a massive efficiency gain, mainly in removing the need for sending tasks back and forth between different professions. The authors of the Green Journal paper conclude that there seems to be a lack of trust in full automation, since the participants in the study made manual reviews of the resulting contours and treatment plans. To me, this is a sound “lack of trust” and having a clinician reviewing and approving the results is as normal as if it would have been a human creating the contours and treatment plans. I don’t see this as a contradiction to the fully automated workflow. As deep learning models evolve, we will need less human *corrections*, but we will still need humans both for review and for developing new treatment concepts. One of the challenges that the authors bring forward is the definition of a “good” plan, since there can be highly competing objectives. Even in the fully automated workflow, there might be a need for producing multiple plans that the clinician can choose from for a specific patient. Some of the plans could have higher priority on the target, others on specific organs at risk. These are questions that we want to address when working on the Plan Explorer module in RayStation, which could be the first point of human contact and where the most suitable plan is selected.
