Lasers alone won’t fix acne scars – here’s why Many people believe that a CO2 laser alone can treat acne scars, but that’s a huge misconception. While lasers are an important tool for resurfacing the skin, they cannot release the tethered bonds that pull acne scars down. For deep, pitted scars, I have to perform subcision – a technique that uses a fine needle or cannula to break up the fibrous bands beneath the skin that are holding the scars down. Without subcision, the skin remains tethered, and even the most aggressive laser treatments won’t fully lift the scars. In this case, the patient had deep ice pick scars that required a combination of advanced treatments to achieve the best results. For this patient, I used a variety of techniques to address the texture, depth, and volume loss caused by his scars. We started with excision for the most severe scars, followed by TCA Cross to target and stimulate collagen in the remaining ice pick scars. To further refine his skin, we incorporated a laser treatment to smooth out the texture. One of the biggest challenges in this case was the significant fat loss in the affected areas, which can occur when scars damage the underlying tissue. Since the patient was very lean and not a candidate for fat transfer, we opted to use Radiesse to improve skin quality and restore volume. Over time, we also added Bellafill, a long-lasting collagen biostimulator, to continue rebuilding lost volume and improve the overall appearance of his scars. At just 29 weeks post-treatment, the results are already remarkable, with the diameter of the scars improving and significant changes in both texture and volume.
