    NOW AVAILABLE IN iQIES: Home Health Preview Reports and Star Rating Preview Reports for July 2025 Hello, friends! Another quarter flying by and our JULY preview reports are in your "My reports" folder in iQIES once again! Don't forget to download them - CMS removes many of these after 60 days and you want to trend your progress! Please feel free to reach out if you have any questions or if I can help you with any of the measures, how they are calculated, or how to continue to improve them! Post-Acute Care Quality Programs: The HHA Provider Preview Reports have been updated and are now available. These reports contain provider performance scores for quality measures, which will be published on the compare tool on Medicare.gov and the Provider Data Catalog (PDC) during the July 2025 refresh. Data contained within the Provider Preview Reports are based on quality assessment data submitted by HHAs from Quarter 4, 2023 through Quarter 3, 2024. The data for the claims-based measures will display data from Quarter 1, 2022 through Quarter 4, 2023 for the Discharge to Community and Medicare Spending Per Beneficiary measures, Quarter 1, 2021 through Quarter 4, 2023 for the Potentially Preventable 30-Day Post-Discharge Readmission measure, and Quarter 1, 2023 through Quarter 4, 2023 for the Home Health Within-Stay Potentially Preventable Hospitalization measure. Additionally, the data for the HHCAHPS measures will display data from Quarter 1, 2024 through Quarter 4, 2024. Providers have until May 2, 2025, to review their performance data. Only updates/corrections to the underlying assessment data before the final data submission deadline will be reflected in the publicly reported data on Medicare.gov. If a provider updates assessment data after the final data submission deadline, the updated data will only be reflected in the Facility-Level Quality Measure (QM) report and Patient-Level QM report. Updates submitted after the final data submission deadline will not be reflected in the Provider Preview Reports or on Medicare.gov. However, providers can request a CMS review of their data during the preview period if they believe the displayed quality measure scores within their Provider Preview Reports are inaccurate.                        How to locate your HHA Provider Preview Reports: 1. Log into iQIES using your Health Care Quality Information Systems (HCQIS) Access Roles and Profile (HARP) user ID and password. (If you do not have a HARP account, you may register for a HARP ID.) 2. From the Reports menu, select My Reports. 3. From the My Reports page, locate the HHA Provider Preview Reports folder. Select View Folder to open the folder. 4. Displayed for you is a list of reports available for download. The reports or files are listed in descending order and the newest files are displayed at the top of the list. 5.     BREAKING NEWS: Telehealth flexibilities have been extended again! Hospice and Home Health FTF telehealth allowed until September 30, 2025. At the last hour in 2024, the government extended the telehealth flexibilities only through March 31st, 2025. The proverbial can was kicked down the road again, and now telehealth will continue to be allowed through September 2025. The Continuing Resolution (CR), passed by Congress on 3/14/25, and then signed by the President on 3/15/25, funds the federal government through September 30, the end of the fiscal year. This Resolution extends telehealth flexibilities that were scheduled to expire 3/31/2025. Most notable, the hospice face-to-face for recertification, as well as the allowance of the home health face-to-face, due to the removal of geographic and originating site requirements, through September 30, 2025. The CR also extends to two percent Medicare sequestration for an additional two months.   • Your Agency’s First HHVBP ANNUAL Performance Report is Ready! Are You UP or DOWN 5%?! On August 23rd, CMS released the first Annual Performance reports, (APR) which provided the first glimpse to agencies of their HHVBP payment impact for 2025. HHVBP's first full "performance year" was CY2023. In the APR reports provided last week in iQIES, CMS provided all agencies their first report where they can now see the CMS projection of the financial impact on all Medicare payments in 2025. From the My Reports page, select the HHA Provider Preview Reports folder NOTE: The folders and reports on the My Reports page are listed in alphabetic order so users may need to utilize the page forward functionality at the bottom of the webpage to advance to the page where the HHA Provider Preview Reports folder is located. Alternatively, users may change the default number of rows that display on the webpage from 10 to a larger number to view the larger list of items. 4. Select the HHVBP file to view the desired report. To quickly locate the most recently published report, select the down arrow adjacent to the Created Date label at the top of the table. This will order the reports in the folder from newest to oldest. 5. Select the file name link and the contents of the file will display.   • NEW RESOURCES AVAILABLE: HOPE Data Collection Timepoints Explainer Video The Centers for Medicare & Medicaid Services is offering a 4-minute, animated explainer video, Hospice Outcomes and Patient Evaluations (HOPE) Data Collection Timepoints for hospice providers. This video describes HOPE, the timepoints, their data collection timeframes, and introduces the symptom follow up visit (SFV), if triggered during a HOPE timepoint. HOPE data collection will be required for the Hospice Quality Reporting Program (HQRP) beginning on October 1, 2025 (or FY 2026).   • BREAKING NEWS FOR HOSPICE: The Hospice CARE Act The Hospice Care Accountability, Reform, and Enforcement (Hospice CARE) Act discussion draft, developed by Rep. Earl Blumenauer (D-OR), modernizes the Medicare hospice benefit through a combination of program integrity provisions and payment reforms to ensure Medicare’s hospice program better meets the needs of terminally ill beneficiaries and their families. Medicare’s hospice benefit has remained largely unchanged since its inception in 1982. Since that time, the industry has changed dramatically – both in terms of the types of providers delivering and the types of patients receiving end-of-life services. Documented, burgeoning fraud and abuse have raised critical questions about how to ensure the benefit is designed in a way that promotes patient access to needed services and safeguards the Medicare Trust Fund. Accordingly, this discussion draft proposes long overdue reforms to Medicare’s hospice benefit to both protect patients and taxpayers from the costs associated with unscrupulous providers and improve access to necessary services and supports for individuals and their caregivers. This discussion draft builds on stakeholder proposals through a series of program integrity and payment reforms that together seek to address many of the concerns expressed by the Department of Health and Human Services Office of the Inspector General, Government Accountability Office, Medicare Payment Advisory Commission, and other government watchdogs over the last decade.
