RN Case Manager Responsible to travel in the community to provide nursing assessments and to supervise and evaluate homemaker/PCA staff Responsible to open new request for service, developing plans of care with the patient and or family, and completing agreements for service Provide visits and ongoing supervision to all patients according to physician orders 1 years prior experience in homecare with Medicare homecare experience a plus Valid RN license in Ohio Current Ohio Driver’s license Health/Dental Benefits, Paid Holidays, Vacations, Sick Leave, Life and/or Disability Insurance
