Last week, I had the privilege of speaking at the South Dakota Academy of Physician Associates conference. It was such an inspiring experience to talk about "zebras" in healthcare – those rare, often-overlooked conditions that PAs should stay vigilant for. What struck me most was the dedication of the incredible Physician Associates in attendance. Many of them are devoted to serving severely underserved populations in rural areas, places that have been abandoned by hospitals and other healthcare providers. These professionals are often the only accessible lifeline of care for people who would otherwise face tremendous travel burdens and hardship to access basic medical services. The heart and commitment of these PAs reminded me of why the profession was created in the first place – to fill critical gaps in healthcare and to bring care to those who need it most. It’s not an easy road, but these individuals are staying true to the PA mission, making a real difference in their communities every day. Medical insecurity in the United States: A deeper look The United States is one of the wealthiest nations in the world, yet our definition of poverty reveals alarming healthcare and living challenges. According to data from Healthcare.gov, an individual is considered at the poverty level if they earn less than $15,060 annually—that's roughly $41 per day. For families, the thresholds are equally concerning: Family of 2: $20,440 Family of 3: $25,820 Family of 4: $31,200 Family of 5: $36,580 Family of 6: $41,960 Now, think about what these numbers mean in terms of day-to-day living costs. Housing, food, water, utilities, and crucially medical expenses, quickly push many beyond their means. A family of six surviving on $41,960 is forced into incredibly challenging decisions about what can be prioritized. Medical insecurity is increasingly becoming a reality for many American families. With rising healthcare costs, even those slightly above these poverty thresholds find themselves struggling to access essential care. This is not just a financial issue, but a public health and social concern that demands the attention of every healthcare professional. I’ll never forget a moment in India. A middle-aged woman I was treating began to cry. Through a translator, I learned it was because she had never received any medical care before. Ever. To reach that stage in life without basic healthcare is both heart-wrenching and eye-opening. But what stood out most was knowing she wasn’t alone—many others I met during that time were seeing a medical professional for the very first time. This paints a sobering picture of medical insecurity worldwide. Disasters and crises heighten the need for an effective response, but it’s clear that addressing global healthcare inaccessibility requires a deeper insight into the individuals and communities we serve. To my fellow healthcare providers, first responders, and global health advocates—this is a call to action. It’s about more than medicine. It’s about understanding, compassion, and connection.
