Effective January 1, 2015, CMS established a non-face-to-face chronic care management service (CCM) as a new Medicare benefit. This service consists of at least 20 minutes on non-face-to-face services that must be completed during a calendar month to beneficiaries with two or more chronic conditions. The finalized CMS CPT code for this service is 99490.
