Chiropractic spinal manipulation reduces opioid related adverse drug events. In their retrospective cohort study evaluating a cohort of 372,471 patients, Robert Trager, DC, Zachary Cupler, DC et al. report: “This study found that adults with sciatica who initially received chiropractic spine manipulation had a lower risk of an opioid-related adverse drug events compared to matched controls not initially receiving CSM, likely explained by a lower probability of opioid prescription. These findings corroborate existing practice guidelines which recommend adding CSM to the management of sciatica when appropriately indicated.” For related CE, go to PostGradDC.com. Thomas et al. offer a new diagnostic support tool for early recognition of cervical artery dissection. The authors report: "This study has assessed and iterated upon an index tool for early recognition of patients with suspected CeAD to provide a validated tool which has promise for use by primary care providers such as general practitioners, physiotherapists, chiropractors, as well as hospital emergency departments...Further validation in Emergency and primary care is needed." Achieving better outcomes depends on communicating in a way that patients clearly understand. #InformedConsent is a process—an ongoing, meaningful dialogue between patient and doctor. When done effectively, it addresses key questions, eases fears, and provides patients with insight into what to expect. Dr. Demetrious evaluated a patient experiencing acute LBP for approximately two weeks. He suspected L5/S1 facet synovitis with localized paraspinal spasm. He explained his diagnosis and recommended #Chiropractic HVLA spinal adjustment. He also reviewed his differential diagnosis, home care, daily activities, potential risks and benefits, possible imaging needs, and alternative care options. Dr. Demetrious outlined what she could expect post-treatment. He informed her that while she might feel immediate relief after the adjustment, acute pain would likely return that evening and possibly persist into the next day before gradually subsiding. To ensure understanding, he had her repeat his recommendations. She consented, and he performed the adjustment within pain-free ranges of motion. Upon rising from the table, she reported immediate relief. A few days later, she returned for follow-up care, reporting about 60% improvement. She thanked Dr. Demetrious for preparing her for the temporary return of acute pain and appreciated the time he took to explain her likely progression. Because he clearly communicated expectations, she trusted him. She valued his proactive approach in easing her concerns and guiding her toward relief. It is with deep sadness that I reflect on the passing of Dr. James Cox. Forty years ago, while still in chiropractic school, I attended a conference where he taught. As a chiropractic radiologist and the developer of the Cox Flexion-Distraction Technique (Cox? Technic), he presented compelling research and objective evidence supporting both his technique and the broader benefits of chiropractic care. His evidence-based approach was groundbreaking at a time when such insights were uncommon in our field. Dr. Cox was not only a pioneer but also a dedicated researcher, prolific author, esteemed educator, and exceptional clinician. Over the years, I would periodically reach out to him for his thoughts on new research, and he was always generous with his time, offering valuable insight and guidance. A few years ago, I had the great privilege of teaching at the annual AMERICAN COLLEGE OF CHIROPRACTIC ORTHOPEDISTS convention, where Dr. Cox’s lecture preceded mine. He was kind, thoughtful, and encouraging—qualities that defined him as both a teacher and a colleague. In 2018, as I prepared my paper on fluoroquinolone degradation as a potential risk factor for cervical artery dissection, I sent it to Dr. Cox for his thoughts. His response was both affirming and inspiring. He described my findings as “the invisible elephant in the room” and urged me to publish my work. His encouragement meant the world to me. Reflecting on his immense contributions, it is clear that Dr. Cox’s work has impacted countless lives—not only through his own clinical and research efforts but also through the chiropractors he inspired. His example is one of the primary reasons I pursued postgraduate education and founded PostGradDC. It is my hope that I can inspire my chiropractic colleagues in the same way he inspired me. Dr. Cox’s legacy will endure through the many lives he touched. May he rest in peace. With heartfelt sadness, James Demetrious, DC, DABCO
