Pioneer Network was formed in 1997 by a small group of prominent professionals in long-term care to advocate for person-directed care. This group called for a radical change in the culture of aging so that when our grandparents, parents — and ultimately ourselves — go to a nursing home or other community-based setting it is to thrive, not to decline. This movement, away from institutional provider-driven models to more humane consumer-driven models that embrace flexibility and self-determination, has come to be known as the long-term care culture change movement. Our partners and audience are primarily engaged in some aspect of long-term care including long-term care CEOs and administrators, consumers and family caregivers, doctors and nurses, direct care providers, and others who care about, and care for, the aging. Pioneer Network is a center for all stakeholders in the field of aging and long term care whose focus is on providing home and community for elders. We believe that the quality of life and living for America's elders is rooted in a supportive community and cemented by relationships that respect each of us as individuals regardless of age, medical condition or limitations. Pioneer Network advocates for elders across the spectrum of living options (which are often dictated by differing levels of the medical care required); and is working towards a culture of aging that supports the care of elders in settings where individual voices are heard and individual choices are respected — whether it is in nursing homes, transitional care settings or wherever home and community may be. Cultivating and maintaining a community of relationships is important at every phase of life, but especially critical for elders and the aging, many of whom may need a network of partners to live life to its fullest.

We're happy to announce that we're working with Presbyterian Villages of Michigan on a grant project designed to replicate the Thome Rivertown model across the country! This unique community in Detroit houses PACE services, Green House homes, and affordable assisted living apartments on the same campus, with all services covered under Medicaid. There's no single payment model for a neighborhood like this, so PVM and its partners have spent years collecting scores of state, federal, and local funding sources to ensure that the community can continue to serve people of all income levels. Funded by Enterprise Community Partners, the grant will allow us to develop a blueprint for a new payment and funding model that will make places like Thome Rivertown the norm, not the exception. Great news from Massachusetts, where Navigator Homes of Martha's Vineyard is making excellent progress on their new Green House community! When completed, Navigator Homes will serve both elders in Green House homes and care staff in dedicated workforce housing. Backed by a USDA loan and supported by the wider Martha's Vineyard community, Navigator Homes stands as a testament to what's possible when dedicated leaders build bridges and seek creative paths to the shared goal of dignified, person-directed care.

The momentum for the EINSTEIN Option just keeps growing! Thanks to McKnight's for covering this bold but eminently possible plan to build full-continuum eldercare services in communities across the country. As Anne Montgomery told McKnight's, older adults "just want to live their lives and be able to tap into and access services when they need them. As their needs change, they want services to be embedded in their own geographic community, and, of course, they want them to be affordable – that’s where the EINSTEIN Option comes in and what it aims to do.”

Today, we welcomed Tom Symondson, CEO of the Aged & Community Care Providers Association, ACCPA to ACH Group’s newest residential care home, Healthia. ACH Group’s, Healthia is the first Green House Project (Center for Innovation: The Green House Project and Pioneer Network), trademarked home in Australia. Tom joined ACH Group, CEO Linda Feldt for a comprehensive tour of Healthia and met with residents and staff to experience first-hand, the award-winning home and small home model of care being delivered at Healthia. Accompanied by Daniela Ciccarello, Bev Malone ACCPA, and ACH Group leaders, Ivy, Marie and Chloe. Residents from Pine House, invited Linda, Tom and the team into their home to experience small home living. The group also visited Healthia’s, state-of-the-art gym where residents work toward their brain health and physical fitness goals. They also experienced the vibrant social atmosphere as residents came together to decorate their home’s Christmas tree. The tour was a great opportunity to highlight how ACH Group’s small home model is working successfully to support people to live good lives.
