Billing and Remittance:?There will be no changes to our Tax Identification Number or other billing and remittance information.?Please continue using the same details for your transactions.?

Insurance Notification:?Kindly inform your insurance carrier of our name change so that a new panel of physicians can be issued. If you would like us to issue you a new panel of physicians, please let us know.?

After-Hours Services:?If you utilize Piedmont Urgent Care at Ingleside Avenue or Zebulon Road during after-hours or weekends, please be aware that a separate invoice will be issued for any services rendered at these urgent care centers.?
