Systematic review found that early weight-bearing after ankle fracture surgery improves ankle function and allows earlier return to daily life in the short term and does not increase complications compared to later weight-bearing. ?? It has been proposed that early weight-bearing after ankle fracture surgery can improve recovery compared to late weight-bearing. This review compared the effects of early weight-bearing versus late weight-bearing on ankle function, time to return to daily life, and complication rates. ?? The review included randomized controlled trials published in English or Chinese, participants of any age recovering from ankle fracture surgery, receiving early weight bearing protocols initiated within six weeks post-surgery or late weight-bearing protocol initiated after six weeks post-surgery. Outcomes included ankle function at 6-, 12-, 24-26- and 52-weeks post-surgery, time to return to daily life, and complication rates. Risk of bias was assessed using the Cochrane Risk of Bias 1.0 tool. Meta-analyses were applied using fixed effects model (if I2 < 50%) or random-effects model (if I2 > 50 %). There was no assessment of the certainty of evidence. ?? 11 RCTs (862 participants) were included. Unclear or high risk of bias due to unclear allocation concealment (5 trials), lack of blinding of participants or personnel (7 trials), unblinded outcome assessors (4 trials). ?? Compared to late weight bearing, early weight bearing showed improved ankle function at 6- (SMD = 0.69, 95% CI: 0.49 to 0.88), 12- (SMD 0.57, 95% CI: 0.22 to 0.92), and 24–26-weeks (SMD 0.52, 95% CI: 0.20 to 0.85) and no difference in ankle function 1 year post-surgery (SMD 0.21, 95% CI: -0.01 to 0.42); shorter time to return to daily life (MD -2.74 weeks, 95% CI: -3.46 to -2.02); no increase in complications (RR: 1.30, 95% CI: 0.85 to 1.98). ??
