Philadelphia Health Partnership is a private, independent foundation. Our mission is to advocate for and resource equitable, quality care and services that improve the health and well-being of people in Philadelphia. We envision a Philadelphia in which everyone, particularly those who have faced barriers to opportunity, achieves optimal health and well-being. The foundation is now celebrating its 25th year in operation. It was created in 1997 as part of the merger between Pennsylvania Hospital and the Trustees of the University of Pennsylvania. Originally known as “First Hospital Foundation,” we became “Philadelphia Health Partnership: Foundation for Healthy Communities” in 2018. Our name stands for our commitment to Philadelphia County, our focus on health, and our philosophy toward collaboration and engagement.

“AFAHO helped me a lot…they helped with my English…my homework…” PHP grantee partner AFAHO is helping to improve the educational and health outcomes of Philadelphia’s African and Caribbean immigrant youth and refugee youth through their African Youth Empowerment Program (AYEP). AYEP empowers youth with educational skills and resources through their mentorship and after school programs. The goal is to improve academic success and engage youth in post-secondary education and training opportunities to help enhance the future of participants. Through the Child Care Voter Campaign, they are spreading the word to a greater audience in key districts and sharing policy goals with candidates and communities.
