Social Determinants of Health (SDOH) are conditions in the environments where people are born, live, learn, work, play, worship, and age that play a significant role in individual health outcomes and quality of life. At PerformRx, our clinical programs provide numerous touchpoints with members, caregivers, and healthcare providers, creating opportunities to address SDOH barriers. For many members, economic instability and challenges in their living environments can create significant health barriers.
