If everyone reading this post were gathered in a large room, I could ask, 'How many of you have at least one toxic employee?' and the majority of hands would go up. A precious few would say, 'I took care of it six months ago, and we’ve never been happier or more productive.' Toxic employees don’t just harm your team’s morale—they create ripple effects that can lead to burnout, reduced productivity, and even impact the families you serve. In pediatric practices, these consequences are magnified because they directly affect the care you provide to your patients. The No A$$hole Rule by Robert I. Sutton is one of the most impactful books I’ve ever recommended to pediatricians. Its central thesis is simple: a toxic employee will cause disproportionate harm to your organization, and the energy spent managing their fallout is energy that could be better spent elsewhere. Creating a thriving practice culture starts with setting clear standards for behavior, modeling those standards, and holding everyone accountable. Hiring and firing decisions shouldn’t just focus on skills—they should reflect your core values. Sometimes, fewer people who align with your culture can achieve more together. Yes, metrics and benchmarks are important, but a thriving practice is more than just hitting numbers. It’s a cohesive, supportive team that shares a common purpose.
